Last Thursday and Friday the Clinical Simulation Program and the Laerdal Medical company hosted a Simulation Users Network meeting at our facility. It was a great success as simulationists from all over New England took part in the activities and interactions.

Tara Landry of Laerdal did a wonderful job pulling the pieces together and supplementing what Cynthia and I were able to rile up with our folks here at UNE and in Southern Maine. It was so great to see so much simulation cooperation. We’d like to especially thank Tara Landry, Chris Scott, Shaun McGovern, Steve Ospina and Lisa Timmons of Laerdal. You guys are quite a team.

And the simulation team from Concord Hospital – Kevin Drew, Christopher Fore, Colleen Rutherford and Marc Desgroseilliers should also be commended for an excellent live OB simulation. We look forward to coming to their center soon!

Attendees "ooohed" when the ambulance pulled up outside with SimMan 3G.

Dr. Chris Bowe of Maine Medical Center’s Emergency Residency Program (along with his residents Hamilton Wells MD and Elizabeth Buyers MD) coordinated with Dawne-Marie Dunbar of UNE Nursing and Patrick Underwood of the SMCC paramedicine program to present a live ED simulation. It was quite exciting! Oh and special thanks to SimMan 3G! He really got run through his paces and passed with flying colors!

Many of the attendees were inspired by the different activities and sessions that were offered. Laerdal gave detailed training on the HeartCode system, VitalSim packages and also offered lots of trade tricks for making moulage, programming SimMan, replacing parts in simulators and they also showed how to navigate their various systems and online databases.

Has it really been MONTHS since I last blogged? Hmmmm…

As you can imagine it’s busier than ever and I’ve included a bunch of new pics to get you up to date with the sim-happenings around here. The summer was really about setting up some new systems, learning new software and fixing and updating equipment. I also finished my ACLS training …which was handy.

So in non-chronological order here are a few pictures and some updates from the last few months.

1. Installed three professional, working head walls into the simlabs for more realism and training options.

Scott St. Pierre of Maine Oxy installing the oxy/air lines to our new head wall systems .

2. Began contracted simulation trainings with Maine Medical Center’s Anesthesiology residency program.

3. I did some simulation consulting work for our friends at Saint Joseph’s College. They’ve built a nice new simlab. Mostly I made recommendations on how things should be set up, which equipment to buy and then trained the users on some of the finer points of running and recording live simulations.

Training to be nurses at St. Joes.

4. Created case study films for Nursing, the Interprofessional Education (IPE) group and the College of Medicine (COM).These type of projects usually entail fleshing out an actual case with actors (faculty and professionals) in a specific setting going through the details with a patient, family member and caregiver. We’ve gotten more artsy with these films and often use multiple camera angles, takes and types and then we edit them with music and titles. We’ll be submitting two films to this year’s Society for Simulation in Healthcare meeting in Phoenix, Arizona. Check the link because they have updated their site recently and it’s more user friendly. Our films from the last two years are also posted there for members to view.

Lori Power as the addicted Mom and Sherril Taub as the pushy nurse in "Critical Encounters".

5. Implemented services from our Mac server. I’ve started the process of setting up services from our big multimedia computer. With Mac OS X Snow Leopard server now installed we’ll soon be able to stream live simulations, create podcasts which can be securely accessed, share large files from afar and have room for critical backup. This system is slowly but surely going to end up being our own tailored data management system.

PA students using the simlab for their Integrating Seminar.

Professor Brenda Beckett in the debriefing room.

A view of the StudioCode program while recording Maine Med's Anesthesiology residents.

6.The PA program is now using the simlabs regularly to do their integrating seminars. One student plays the patient while the others (in groups of 2) assess and provide care to the patient, while formulating a diagnosis. Other PA students and faculty watch from the debrief room and everyone gets to see and hear themselves(they cycle through) with the AV and recording system provided by the Clinical Simulation Program (CSP).

7. The School of Social Work has begun using the simulation labs as training ground for students. Adjunct professor Cynthia Tack created some hybrid cases for live simulation use and broadcast. Like with the PA’s and other folks we work with, the simulations are recorded and replayed in the debriefing room for discussion.

8. Showcased an interdisciplinary case study film created by the CSP and IPE for the 09 College of Health Professions Orientation. Hundreds of new students got to work on a case study with peers from other disciplines. The guided discussions followed the film-which was an interview with an Iraq War veteran who had multiple social, mental and physical ailments.

Social Work simulations in progress.

Of course we are running simulations as usual this semester for various Nursing, MMC ED and MNA classes and have plans to work soon with our new College of Pharmacy.

Thanks for tuning in. Blog ya later….

Now that we are stationed on two floors, Cynthia and I keep getting our exercise running between them! After a terrifically busy semester we’ve got some good data and experience to work with for future improvements to the labs and structure of our administration. Mostly we’ve got to keep fine tuning our organization so that as a two person program we make processes easier while the simulation use hours go up. Eventually we’d like to digitize our log-in process as well as the evaluations and confidentiality forms. For now we are still using pen and paper and asking folks to fill the required forms out before and after simulations. Of course, in the big picture we see going to an entirely full featured data management system which allows all the data we are generating, sending and receiving to be housed in one place that can be accessed from anywhere. My hope is that in a year or so we’ll be implementing this type of hardware /software combination for the betterment of all the programs we work with and for the Clinical Simulation Program(CSP).

Central Maine Medical Center (CMMC) donated an anesthesia machine and monitor to us (MNA Program and CSP) in April. And we thank them! Once I get it all rigged up and connected to the METI HPS rack and it spits out the right numbers, we’ll have two independent simulated OR rooms. The picture below shows that once you start a bigger project like this you might as well fix a bunch of other ticks that have cropped up. As you can see that means changing out speakers, putting on a new chest skin, checking connections etc.

HPS getting a makeover and new-old anesthesia machine

Several simulations this year have included actors and actresses (faculty and staff mostly) playing key roles during a case. As part of this growing sim-trend we’ve had screaming, emotional wives, uncooperative, inebriated interns and as below…bad guy boyfriends who shake babies and intimidate nursing students!

Pretending for a "Families in Crisis" nursing sim with pal Brenda

With more space to work with the MNA program can now host more anesthesia equipment vendors during their ” Difficult Airway Days”. The room that students are in (in the picture below) can house the whole MNA class as well as four anesthesia machines, room for SimMan, a SMARTboard, fiber-optic scope setup and storage spaces.

MNA students checking out the latest in hi fi laryngascopes

The MNA program also launched it’s first pediatric scenario using the SimBaby. The case was based on a very scary situation that a former student went through. The case gave students a feel for dealing with a baby in distress while doing so in a safe environment where they could reflect and learn. Running this scenario has opened the door for more anesthesia baby simulation ideas in the future.

SimBaby in the 242 theater, about to "see" students

The Dental Hygiene Program used simulation to teach proper procedures and protocols for emergencies in a dental clinic. Students dealt with patients that were not easily manageable and some that had medical conditions that had to be considered before treatment.

The Dental Hygiene program using the new debriefing space

The Physician’s Assistant program used the Laerdal Heartcode ACLS system through the CSP to study, test and become certified ACLS providers. The system provides students with case scenarios on an interactive CD ROM that is similar to a video game. After they pass the cases and make the right decisions they come to the lab to do their skills tests. Forty-plus students got their ACLS cards via Laerdal and the AHA. Pretty cool…….

PA students testing their CPR and ventilation skills

There are quite a few plans for the summer. Mostly we’ll be upgrading our labs with new headwall systems and I’ll be installing AV connections in two more rooms.

Thanks for “listening”.

Anne Summer, Jim Behme and I just got back from our sim-consulting trip to the University of Medicine and Health Sciences (UMHS) in St. Kitts! Anne and Jim both work at the Clinical Performance Center on the Biddeford campus of UNE. We were asked to consult with the UMHS faculty and present our expertise on high fidelity simulation and standardized patients. The school faces the Carribean Sea and is an inspiring place to house a higher learning facility. We met with quite a few faculty members and jostled with ideas to embed simulation into their collective curricula. It was a successful series of meetings and much information was traded back and forth. The simulation labs are spacious and can accommodate many students at once.

The view ain’t so bad either!

Under the guidance of nursing instructors Helen Gillanders, E.N.(G), R.N.(G) and Verena Pointer R.N. we got a lovely tour and demonstration of their Human Simulation Laboratory.

Ellen Gillanders with nursing students and SimMan.

Mrs. Gillanders was initially educated as an enrolled nurse (G) at Sheffield School of Nursing in England. She worked in the regional kidney transplant unit at Manchester Royal Infirmary that contained a hemodialysis treatment area and a training area for patients undergoing peritoneal dialysis. Mrs. Gillanders also worked in the regional vascular surgical unit attached to University of Manchester Medical School with responsibilities for coordinating the administration of medications under research protocol in conjunction with the University of Manchester Medical School. Later, she completed additional education to become a registered nurse at the University of Manchester Nursing School. She was one of the primary health care providers at Ross University School of Veterinary Medicine.

Verena prepping for simulations!

Mrs. Gillanders is a clinical instructor in the skills laboratory at IUON, and she is the manager of the exceptionally well equipped state of the art IUON Human Simulation Laboratory. In addition, Mrs. Gillanders currently serves asthe IUON and UMHS campus nurse. Mrs. Pointer received her nursing education at the German Hospital School of Nursing in London. After qualifying in 1967, she was in charge of a 12-bed ear, nose and throat unit (ENT) for four years. Since then she has travelled the world with her physician husband working on various international aid projects.
Mrs. Pointer was associated with Ross University School of Medicine since 1996 when she and her husband moved to Dominica, West Indies. Mrs. Pointer joined IUON in January 2006 as the human patient simulation lab assistant and enjoys the interaction she has with students and faculty.

Here are more pics from the trip:

Setting up for a vent-assisted simulation.

It’s not yet Spring but it’s starting to feel like it’s around the corner. Of course in Maine we have to be truly patient as it doesn’t usually get “warm” until mid April. Things are heating up here in the simlabs these days too! We have been operating the new upstairs lab and debriefing spaces for about 9 months now and it’s been a great expansion. We’ve been able to service more individual program needs and control student and faculty flow through the simulation experiences.

Here’s the power list of some of our accomplishments since I last blogged …which was way back in November!

A nursing class has to deal with an asthmatic kid and a Mom who smokes.

1. Created several learning module movies for Anesthesia, MMC ED and Social Work classes. These films are created from scripts in which faculty act out certain roles in a simulated experience then the video gets cut and merged and edited down to a movie which can be played for multiple classes and presentations.

Showing the pedi HAL simulator's abilities to interested legislators.

2. Presented and demonstrated with simulator to almost 100 freshman Maine legislators.

3. Programmed several new cases on the METI HPS with Dr. Chris Bowe and Dr. Jeff Holmes for their ED resident simulations.

Dr. Chris Bowe preparing MMC ED residents for simulation.

4. Programmed and supported Maine Medical’s Internal Medicine (IM) department with their first round of emergency simulations. MMC IM just recently started up with us and will continue with simulations in the near future. MMC Nursing is expressing a strong interest in using simulation to teach as well. The future looks bright for continuing connections with Maine Medical Center.

Happy nurses after finishing refresher course.

5. CSP Coordinator Cynthia Morris taught part of a Nurse Refresher Course recently with simulation. Nurses from around Maine come to the labs and freshen up on nursing skills.

6. I visited the incredible simulation center at the Gordon Center for Research in Medical Education (GCRME) at the University of Miami. This is where the world famous Harvey simulator was created and is still manufactured. I was taken through the magnificently equipped center by Vivian Obeso MD who is the assistant director and clinical skills trainer. The Gordon Center has a ton of space to work with and they also use it wisely. It’s also very sleek and stylish like the rest of Miami!

Here’s a few pix from the University of Miami’s Gordon Center Simlab:

Vivian Obeso demonstrating the Harvey simulator.

7. I also presented at the International Meeting on Simulation in Healthcare in Orlando and it was a great experience. Here’s a slideshow from the conference. I got excellent feedback about my presentation on how we use the Laerdal AVS system and had some good interactions with fellow filmmakers during the film sessions. I saw some amazing new simulation technology (some of which I’ll be featuring in the next blog) and interesting presentations. All presenters materials will be on the SSiH site soon. So I’ll post that when it happens too.

The conference was at the Contemporary Resort in Disneyworld and my wife Nancy and son Uther also came along! I’d never been to Disney before . It was awesome to experience it through the eyes of my six year old boy!

See pics below!

8. Of course we have continued with running simulations for our excellent Nurse Anesthesia program including more difficult airway sessions, induction sequences, machine checks and Crisis Resource Management scenarios. The Nursing and PA programs have also increased their use of the labs and the PA’s in particular are taking their ACLS course through us with the Laerdal Heartcode system. They will get their certification cards after completing the microsimulations and passing their CPR exams. So far, so good.

9. I am leaving for St. Kitts this Friday to join Anne Summer and Jim Behme from UNE’s Clinical Performance Center. We have been asked to visit with the folks at the University of Medicine and Health Sciences in St. Kitts…in the Caribbean! Needless to say we are very excited about this trip and eager to share the knowledge we have in our specialties of standardized patients and high fidelity simulation. We will be taking pictures of their beautiful facility and lots of this little paradise so look for those in the next blog!

Thanks for reading and look for a new format to this blog by next month!

It is a sad week here at the University of New England. We have lost a dear friend, great educator and champion of the simulation program. Nona Spear lost her battle with cancer and passed away last Saturday. Services were held this week in Windham and South Portland Maine. Nona was the original simulation guru on campus and set up the procurement of the METI HPS and Laerdal SimMan simulators on the Portland campus four years ago. She had a strong hand in hiring me as a technician, training me and getting me up to speed with what I’d need to know medically and where the position would lead. I will never forget the chance she gave me and will not let the fire she lit go out.

From all of us here at UNE, I think I speak the truth when I say that there will never be another Nona Spear. We will miss her hearty laugh and powerful personality. No one cared more about students doing things the “right” way than she did. She will be missed dearly. Her spirit lives on in all of her friends at the University of New England. ————————————————————————–

Per usual there are many updates to highlight since last I blogged. Let’s get right to it.

Medical gas wall outlets!

-The Blewett 116 and 117 simlabs now have pressurized medical O2 from wall mounted hookups. These directly connect to the anesthesia machines in the rooms. This upgrade improves the efficiency and reality of the OR and ER simulations in these rooms.

- The SMARTboard install is complete for Blewett 203. The room just became a whole lot more versatile. Debriefings with Laerdal’s AVS system are possible with the touch of a finger. We also now have our regular CSP Users Group meetings in there and it will be a viewing room for completed video projects as well as a hub for brainstorming sessions.

Nursing students viewing simlab action through SMARTboard.

- CSP Coordinator Cynthia Morris has been uploading a ton of learning materials to the UNE Clinical Simulation Program Blackboard site. She has designed it to be a resource for our faculty and students. We will be continuing to expand this distance learning module throughout the coming months. For our sim users who have access to Blackboard here is the starting point. Have a look at the multiple materials at your fingertips.

-Professors Nina Turcato and Cliff Roberson of the Nurse Anesthesia program are presenting at the HPSN conference that METI hosts every year. This year in Tampa they are presenting: A Developmental Approach to Simulation in Nurse Anesthesia Education. Description:The University of New England’s School of Nurse Anesthesia faculty will describe their experiences in creating a developmentally appropriate simulation-based curriculum. The integration of simulation into the program occurred over a four year time span. The presentation will emphasize the course of faculty training in simulation-based learning techniques, as well as the creation of simulation modules appropriate for both the pre-clinical and clinical phase learner in the 27 month program. Curricular goals focus on technical and non-technical skills as well as the promotion of student reflection using an experiential learning model.

-I will be representing UNE as a presenter at the International Meeting on Simulation in Healthcare in January. Our accepted video presentation is entitled “Emotive Immersive Transformative Simulation”. This video was created from footages gathered during an interdisciplinary simulation and composed in iMovie. The video might be streamed from the ssih.org site after the conference. I’ll let you know.

“They’re Back!”

The students are back and our newest simulation labs are ready to go! As we are digging into the new semester it’s fun to look back at some of the simulation projects we’ve tackled. This past summer was especially busy with the new labs getting finished, MNA’s Crisis Resource Management simulations happening, and Maine Medical’s ER residents running through 12 new scenarios created by Dr. Chris Bowe. One simulation in particular created quite a stir on campus.

Multidisciplinary simulation on the green!

Writing and presenting

We are proud of our Nurse Anesthesia faculty as they have truly taken simulation education to the next level in the past year. UNE’s MNA students are getting a 5 to 1 faculty to student training experience in our labs working with the METI HPS simulation system and using state of the art anesthesia equipment. Recently Nina Turcato and Cliff Roberson were published in the August 2008 AANA Journal where they shared their findings about simulation based education for Nurse Anesthetists. Here is the article in full- simulation-based-education-aana-article-une

Cynthia Morris-Associate Professor of Simulation Education-is taking a very interesting course at the Mayo Clinic Multidisciplinary Simulation Center called Instructor Development: Simulation-Based Education Design and Debriefing.

I will be speaking at the annual Society for Simulation in Healthcare meeting this year. As part of the Operations Track I’ll address IT needs and focus on streaming audio and video on a gigabit network. Here’s the Ops Track Agenda-imsh-ops-track-2009vtd

New labs up and running…

The Nursing Program christened our new simlabs and we all really enjoyed working in our new environment. While we are still awaiting a few smaller installs and the 2 way mirror, the technology is in place and working marvelously. Both SimMan and SimBaby are functioning and the new assessment/skills training/debrief room is a wonderful new addition to our program. Check out the newest pictures!

Nothing keeps you busy throughout the summer like building another simlab! Presently we are about a month away from being totally set up. The plan is to separate the Laerdal and METI equipment to better streamline activities and also to better take care of our growing demand from the different programs we assist. We’ve simply needed more space for our growing program. So SimMan, SimBaby and Nursing Anne are now on the next floor up in a new lab which also houses our new office and second control room.

We’ll also now have a place to run our Heart Code ACLS certification package. This is a certified ACLS learning and testing program that allows folks to sign up and run through the ACLS tests over multiple hours. It’s a very well conceived program that actually does a lot of the work over the internet. When a student passes – it sends the info to the American Heart Association and they send the student their ACLS certification card! We will officially launch this in the fall of 08.

Laerdal's Heartcode ACLS Anywhere training package.

7. Two way mirror for viewing from office to lab.

With the expansion to the upstairs it’s creating more room for our 2 METI HPS simulators and the anesthesia machines in our downstairs labs. This installation is happening soon too. The virtual OR and ER are getting a little more “real.”

Big/Little purchase: Well, we bought a Gaumard pediatric HAL simulator and we love him! He is my new favorite simulator to use for those impromptu demos that happen in the summer! He can be turned on in 1 minute and with his totally wireless capability, blinking eyes, superior sounds and pulses… he is a hit already.

He's totally wireless and so is the monitor behind him.

We’ve already used him in our hallway for a few scenarios and have plans to do a larger scale simulation with SMCC’s CCEMT program, ER residents and possibly passers by. I predict that Gaumard will sell alot of these simulators and so far I am totally impressed with the fidelity, usability and medical validity of this kid. Looking for a new simulator? Check out Gaumard. They are rising fast.

So many tasks still remain but progress is happening. This next year looks to be our busiest yet and with the planning we’ve done it should be our most successful. We’ll probably have some type of open house for the new simlab soon and will probably post pictures in the next blog.

Stay Tuned.

Another two months have passed! And again, we have been expanding our simulation activities.

PA students called to a code in the simlab during the ACLS course.

HIGH FIDELITY ACLS COURSE

On April 17th and 18th we provided the PA program with the use of all of the simulation labs for a two day Advanced Cardiac Life Support course. ACLS instructor Doctor John Alexander-who lectured and taught with simulation-created the program with the Clinical Simulation Program staff.

48 students followed a nationally certified program model with the added enrichment of a dedicated high fidelity simulation experience threaded throughout. Cases such as Symptomatic Bradycardia and Pulseless Electrical Activity were run on high fidelity METI and Laerdal simulators and then diagnosed and treated clinically by the students.

PA students practicing the 2008 protocols for chest compressions.

At times lectures were interrupted by mock codes that were called on the fly! Students were then cast into makeshift teams and shuffled into the simlab to treat a serious ACLS case. Several extra faculty and adjuncts were employed to present this important learning oppurtunity.

MAINE SUN MEETING

Shaun McGovern of Laerdal teaching some “sim-tricks” with the ALS simulator.

On May 16th and 17th we partnered with international simulation company-Laerdal-to host a Maine Simulation Users Network (SUN) meeting. As Maine is nationally ahead of the educational curve when it comes to teaching with simulation, it seemed appropriate to finally have the meeting here on UNE’s Westbrook campus and use our simlabs and environment. Attendees were from all over New England and were engaged and enthusiastic throughout the day and a half get together.

Clinicians, educators and technicians watching the live simulation.

Simulation ideas, research findings, tricks and stories were shared and a live nursing simulation was simultaneously broadcast to the class where the thirty plus clinicians and teachers were. Laerdal’s Advanced Video System (AVS) was synced to the SimMan program and allowed the “watchers” to see 3 camera views, hear the sound, view the dynamically changing patient monitor and the case log as it unfolded.

Instructor Dawne-Marie Dunbar debriefing students after the simulation.

Below is the agenda for the 2 day event:

SIMULATION PRESENTATION FOR KENNEBUNK HIGH SCHOOL

Recently Cynthia Morris and I presented to students from Kennebunk High School.

Here is the link to the story which was posted through the UNE News webpages.

NEW HORIZONS!

The summer ahead looks even busier as we are implementing new simulators, new debriefing systems, high definition video capabilities and building our new simlabs and second control room. We will also be piloting a few new courses, taking our new pediatric simulator to kindergarten and upgrading our gas delivery systems. Please stay tuned for more of the latest happenings.

Thanks for reading on.

4. I will be presenting at “Beyond the Limits”- an exciting EMS focused conference at the Kalahari Resort and Convention Center in Sandusky Ohio from May 8-10th. The event is being put on by EHOVE – and I’ll be speaking on what it takes to start up and run a high fidelity simulation lab. My talk will have a fun yet very technical bend to it, focusing on the growth we’ve gone through and what the Clinical Simulation Program has learned along the way. My next blog will have the link for the event and more information. ——————————————–

So congratulations to our simulation speakers and collaborators.
